Please read this document carefully. By signing this document, you are waiving certain legal rights, including the right to sue [A Connected Touch Massage Therapy LLC/ Marcus Dunning ] and its staff.

1. Description of Activity

I am voluntarily participating in a sound bath / sound healing session hosted by [A Connected Touch Massage LLc]. I understand that a sound bath involves lying or sitting down for an extended period while being exposed to high-volume sound frequencies and vibrations generated by instruments including, but not limited to, gongs, crystal singing bowls, Tibetan bowls, chimes, and drums.

2. Acknowledgment of Medical Risks & Contraindications

I understand that while sound baths are generally considered safe, intense acoustic vibrations travel physically through the body and can affect certain health conditions. I acknowledge that it is my sole responsibility to consult with a physician prior to participating if I have any medical concerns.  

Specifically, I am aware of the following conditions that require caution or prior medical clearance:

• Cardiac Pacemakers / Defibrillators / Stents: I understand that sound vibrations (especially from instruments placed directly on or near the body) can potentially interfere with electronic medical devices or resonate with metal implants.

• Pregnancy: I understand that intense, low-frequency vibrations are generally discouraged during the first trimester or in the final weeks of pregnancy, and I assume all risks related to participating while pregnant.  


• Epilepsy / Seizures: I understand that certain sound frequencies or sustained rhythmic beats may trigger individuals with sound-sensitive epilepsy.

• Hearing Aids: I understand that high-frequency sounds may cause high-pitched feedback or discomfort in hearing aids, and it is recommended to adjust or remove them during the session.  


• Mental Health Conditions: I understand that deep relaxation and sound frequencies can sometimes release suppressed emotional material or cause disorientation for individuals experiencing severe PTSD or acute anxiety. 


3. Assumption of Risk

I knowingly and freely assume all risks, both known and unknown, associated with my participation in this Activity, including but not limited to physical discomfort, emotional distress, changes in blood pressure, dizziness, hearing sensitivity, or aggravation of pre-existing conditions. I assume full responsibility for my own physical and mental well-being during and after the session.

4. Release and Waiver of Liability

In consideration for being allowed to participate, I hereby release, waive, acquit, and forever discharge [A Connected Touch Massage LLC], its owners, practitioners, employees, volunteers, and independent contractors (collectively, the "Releasees") from any and all liability, claims, demands, or causes of action arising out of any physical or psychological injury, economic loss, property damage, or death that I may suffer as a direct or indirect result of my participation, whether caused by the negligence of the Releasees or otherwise.

5. Indemnification

I agree to indemnify, defend, and hold harmless the Releasees from any and all claims, suits, actions, or expenses (including attorney's fees) brought by me or anyone on my behalf arising out of my participation in this Activity.

Participant Acknowledgment & Signature

By signing below, I certify that I am at least 18 years of age, have carefully read this Waiver and Release of Liability, fully understand its contents, and freely and voluntarily agree to its terms.

6. Media and Photography Release

I hereby grant [A Connected Touch Massage Therapy LLC ] and its legal representatives the irrevocable and unrestricted right to take, use, reuse, publish, and republish photographs, video recordings, or audio recordings of me taken during the Activity (the "Media").

I understand and agree to the following conditions regarding this Media:

• Purpose: The Media may be used for any lawful purpose, including but not limited to promotional materials, social media, marketing campaigns, website content, and educational resources.

• Compensation: I waive any right to inspect or approve the finished product, advertising copy, or print/digital material that may be used in connection with the Media. I understand that I will receive no financial compensation or royalties for the use of these images or recordings.

• Release of Claims: I hereby release and discharge [A Connected Touch Massage LLC ] and its representatives from any and all claims and demands arising out of or in connection with the use of the Media, including without limitation any and all claims for libel, invasion of privacy, or right of publicity.
